The following legal documents have been provided to assist you in receiving the various services that the High Commission and the Consulate General perform on behalf of the Government of Trinidad and Tobago. These forms can be printed, completed by hand and mailed to the High Commission or Consulate General or brought in person with the appropriate fee and documentation for processing.
Birth Certificate Form
A
birth certificate is a document which is used, among other things, to establish legal identity
and will be of use to obtain
health services, a passport, an old age pension and many other
beneficial services.
A Computer-generated Birth Certificate has been introduced, and will be offered free of charge for a first time issue on compliance with the stipulated requirements. A diplomatic service charge of Can$3 payable by Money Order to the High Commission is applicable.
The old style Certificate remains valid however and therefore applications will continue to be processed. A diplomatic service charge of Can$3 payable by Money Order to the High Commission, and a processing fee of Can$4 payable to the Registrar General are applicable.
PLEASE NOTE that it is mandatory to submit a Computer-generated Birth Certificate in order to obtain the Machine Readable Passport.

Certificate
of Character Form
This form is for persons
who are applying for a Police Certificate of Good Character from the Criminal Investigation
Department/Criminal Record Office of the Trinidad and Tobago
Police Service. Police Certificates of Good Character verify
that the applicant has no previous convictions against him/her.
Certificates of good character are often required by persons
seeking to obtain permanent residence or citizenship in Canada.

Death Certificate
Form
Death registrations are a permanent, legal
record of the death of an individual. This form is used by individuals
to
request proof of registration (certificates, certified copies)
of death occurring in Trinidad and Tobago.

Renunciation of Citizenship - Form 13
Some citizens are required to renounce their Trinidad and Tobago citizenship for various reasons e.g. to acquire citizenship of another country that insists on this stipulation. The Declaration of Renunciation of Citizenship Form is to be used for this purpose.
Restoration
of Citizenship - Form 16
This form is used by individuals to restore their Trinidad
and Tobago citizenship status. Medical Forms P&I 40 and P&I
40A must also be completed.
